Order to build a form (test) on the website.
I am looking for a person to make a form on a website, similar to the one available under google forms.
The form should be editable (I can handle the html file).
The form should be divided into sections. All sections will be closed questions, except one open question.
To illustrate;
Section 1
Question 1: How often do you use your computer.
Answer: Never, once a month, several times a month, once a week, several times a week, daily.
Question 2 How often do you play computer games.
Answers: Never, once a month, several times a month, once a week, several times a week, daily.
A point value will be assigned to each answer.
Each section should generate a point total based on the answers given. The person surveyed will not see it.
After going through each section and "submitting the form". The form, i.e. the values of the questions given (similar to google form), should be sent to my email/server.
E.g. Section 1 - score 18pkt
Section 2 - score 22 points
And so on.
Access to the form should preferably be secured with a token/code. The form will be paid for. Once purchased, the person will receive an access code. Ideally, the code would expire after a single use.
